The iPACK Block is a regional anesthesia technique that provides analgesia to the posterior capsule of the knee. iPACK stands for Infiltration between Popliteal Artery and Capsule of the Knee. An iPACK block can be beneficial for:

  • Knee Arthroplasty
  • ACL Repair
  • Knee Manipulation
  • Post-Operative Knee Pain

Overview of the iPACK Block

The iPACK block provides analgesia to the posterior knee capsule by targeting the articular branches from the sciatic nerve and the posterior branch of the obturator nerve. Moreover, the block can be performed with an adductor canal block and ultrasound-guided LIA block to provide additional analgesia to the anterior knee capsule.

Risks and Side Effects

Any procedure that involves anesthesia comes with its own set of risks. Some risks and complications may include:

  • Infection
  • Allergic reaction
  • Nerve injury
  • Intravascular injection/ Local anesthetic systemic toxicity (LAST)
  • Hematoma

How to Perform the iPACK Block

To perform the block, place the patient with the operative leg externally rotated and slightly flexed at the knee or in the prone position. Next, place the ultrasound transducer on the posteromedial thigh approximately 1 to 2 fingers proximal to the patellar base. Identify the femoral shaft and the popliteal artery. The surface of the femur will appear as a hyperechoic line deep to the popliteal artery. Insert the block needle through the skin about 2-3cm medial to the transducer and advance in-plane using a medial to lateral direction. The needle is advanced within the interspace between the popliteal artery and femur until the needle tip extends 2cm beyond the lateral border of the artery.
